In order to apply for the Polish passport, a certificate of Polish citizenship is required as well as the registration and issuance of personal documents: birth certificate and/or marriage certificate and/or name change certificate in Poland.
Our office assists in the registration and issuance of the certificates at the Office of Population Registration in Poland ( USC ).
An application for the issuance of a Polish passport can only be submitted in person at the Polish consulate in Tel Aviv.
